Replacement of Dilapidated ID Card
Requirements: Certificate of Registration; Dilapidated ID
Location: Registrar’s Office of the Campus
Fee: P230.00
Steps:
- Present the requirements to the Registrar’s personnel. The client will be issued with ID Replacement Form. The Replacement Form is valid for five days to allow students to enter the campus until his/her ID is processed. ID processing is scheduled every Thursday at the MIS Office (Main Campus).
- Pay the ID Fee at the Cashier’s Office. Get the official receipt (OR) of payment.
- Proceed at MIS Office at the Main Campus and present OR and ID Replacement Form.
- Receive the ID Card.
Issuance of Transfer Credential
Requirements: Duly signed clearance form; one (1) piece documentary stamp
Location: Registrar’s Office of the Campus
Fee: P60.00 per page
Steps:
- Present duly signed clearance to and secure request form at the Registrar’s Office.
- Fill out and submit the request form to the Registrar’s personnel.
- Pay the Transfer Credential Fee at the Cashier’s Office. Get the official receipt (OR) of payment.
- Submit the request form with official receipt, and documentary stamp to the Registrar’s personnel.The Registrar’s personnel will prepare the Transfer Credential. The release of Transfer Credential usually takes thirty (30) minutes.
- Receive Transfer Credential and sign the log book.
Issuance of Statement of Accounts
Requirement(s):School ID or Certificate of Registration (COR)
Location: Cashier’s Office of the Campus
Fee: P50.00 per copy
Steps:
- Present school ID or COR to the cashier. The cashier will validate the presented requirement. If the requirement is valid, the cashier will print and sign the copy of the client’s statement of account.
- Pay the certification fee.
- Receive the printed Statement of Account.
Refund for Dropping a Program (rev. 2012)
Requirement(s): Duly accomplished dropping form signed by the Dean and the Registrar; Official Receipt; Certificate of Registration
Location: Cashier’s Office of the Campus
Fee: None
Steps:
- Present requirements to the Cashier’s Office. The cashier will validate the presented requirements based on the existing policies on refund.
- After validation, you will be required to return on the date of release. The preparation of refund usually takes 3 working days.
- On the scheduled date, go to the Cashier’s Office and receive the refund. In case you are sending a representative, authorization letter and valid ID of representative will be required.
Issuance of Library Card (rev. 2012)
Requirement(s): Certificate of Registration and two pieces 1x1 photos (not computer generated, with white background

Steps:
- Present requirements to the Library personnel. Blank student information sheet and library card will be issued to you.
- Fill up student information sheet and library card, then submit to the library personnel.
- Receive the Library Card.
- Sign the log book.
